Part 2
Planning Data for Future Services

Information in this survey, like dokusan discussion, is confidential, that is to say only the Shuso or head monk has access to the details. Totals and averages are used to determine what is feasible and what is not for the upcoming 5 year planning period. Please be as blunt ( my skin is pretty thick!) and accurate as possible so that we do not plan events which are not utilized and find ourselves unable to finance them.

Of the events and services listed above, which are you planning to make use of during the upcoming year?

Part 3
Support

This is where the rubber meets the road or where generous intention meets (or doesn't) actual action. Again please be as accurate as possible. It's better to pledge 5 minutes and $5 and actually make sure you end up on a meditation cushion / it ends up in the donation bowl then pledge a large time / sum and not carry through.

Please place a D - daily, W - weekly, B - biweekly, M - monthly Y- yearly beside the appropriate categories.

I pledge the following support:

Event Daily Weekly Biweekly Monthly Yearly
Zazen meditation on the zafu cushion.
Zen practice in the context of the work a day world.
Assisting with weekly services - Zendo soji & set up/take down (cleaning and maintenance).
 Providing altar flowers.
Providing tea or snacks for public presentations.
Assisting with planning / public relations for events.
 Web site updating.
